COVID‐19 PANDEMIC ‐ PATIENT DISCLOSURES

 

In order to provide a safe environment for all patients and staff, we ask that you take a moment to answer these COVID-19 screening questions. 

If you answer YES to any of the questions below, please call our office at 972-234-8800 to reschedule your appointment.

1.  Do you have a fever or above normal temperature?

2.  Have you experienced shortness of breath or had trouble breathing?

3.  Do you have a dry cough?

4.  Do you have a runny nose?

5.  Have you recently lost or had a reduction in your sense of smell?

6.  Do you have a sore throat?

7.  Have you been in contact with someone who has tested positive for COVID‐19 or is awaiting results?

8.  Have you tested positive for COVID‐19?

9.  Have you been tested for COVID‐19 and are awaiting results?

10. Have you traveled outside the United States by air or cruise ship in the past 14 days?

11. Have you traveled within the United States by air, bus or train within the past 14 days?

New Dallas County order requires us to enforce Face Mask for patients.  Please wear a face mask upon entering our office and sanitize your hands with the hand-sanitizer at the counter. Face mask will be lower during dental procedures. Be ready to answer the above questions again. We will check your temperature with our contact-less thermometer before checking you in.  Please come by yourself and do not bring family members or friends. If your child is under 18 and has an appointment, a parent can accompany the child.
